Case History

The patient had previously sought treatment at a familiar clinic, where root canal procedures were performed on all her incisors. It is not sure why her chief complaint of bleeding gums and swollen gums was completely ignored. However, her symptoms persisted, leading her to seek further assistance from Dr. Swetha U.

Examinations

In the initial clinical examination, her gums were reddish in color, would bleed just by touching. Thorough clinical and radiological examinations, including an OPG Full Mouth X-Ray were undertaken.

Diagnosis

The Digital OPG X-Ray which was taken uncovered the underlying issue: faulty root canal treatments in all upper and lower incisors. Dr. Swetha and her team were left completely surprised and heartfelt at the condition of the root canals.

Consultation

Dr. Swetha took her time with the patient by explaining her what needs to be done. She took the time to slowly expose the fact of the botched root canals. Upon hearing about the botched root canal treatment, the patient was completely in tears. Dr. Swetha and her team consoled her repeatedly and assured her that they can fix everything. This case was going to be a unplanned and unnecessary smile design treatment. The consultation took more than 2 hours, but keeping the patient’s plight in mind, Dr. Swetha stuck to her and laid out a simple but effective treatment plan.

Treatment Approach

Dr. Swetha U devised a comprehensive treatment plan tailored to the patient’s unique needs. Initial steps included scaling, root planing, and laser periodontal therapy to address the bleeding gum issues. A thorough session on how to maintain oral hygiene was given to the patient including how to brush and how to rinse. A review was undertaken after 15 days. Not surprisingly, the Laser treatment has worked wonders and conscious maintenance of the oral hygiene by the patient resulted in complete stoppage of the bleeding from the gums.

When Dr. Swetha checked her and confirmed this, the patient was overjoyed and ecstatic By now she had pulled up the courage to go for the second leg of the treatment, which was Re-Root Canal Treatments. The Re-root canal treatment were performed on the affected teeth (11,12,21,22,31,32,41,42). Dr. Swetha and her team, took their time with the Re-Root Canal Treatments and ensured they got it perfectly. All the Re-Root Canal Treatments were completely successful. This was followed by the placement of zirconia crowns to restore both functionality and aesthetics.

The patient’s satisfaction with the treatment outcome was evident, as all dental and gum issues were successfully resolved, culminating in a corrected smile. A follow-up schedule of once every six months was established to ensure the long-term maintenance of oral health.

1. Dental Implant
Who is eligible for dental implant? Any age group is eligible for dental implants unless that person has healthy gums and underlying bone & except the age group is below 18 yrs.

What is Dental Implant? Dental Implant is a titanium screw placed directly into the jawbone to replace and restore the functionality and appearance of the missing tooth. It supports the crowns, bridges, dentures etc. Dental Implant consists of a dental implant body, an abutment and may also include an abutment fixation screw. It is usually made up of titanium or zirconium oxide others like gold alloys, cobalt-based alloys, or ceramic materials. Bio-compatible and does not cause any irritation or allergic reaction and are safe.

How is an Implant Placed? Dental Implant is surgically inserted in the jawbone in place of the tooth’s root. The abutment is attached to the implant body by the abutment fixation screw and extends through gums into the mouth to support the artificial dental crown. Vital structures (like maxillary sinus, mental nerve & inferior alveolar nerve) near where implant is to be placed should be evaluated carefully by CBCT during surgery. After implant body placement, 3-6 months’ time period is given to the implant and bone to undergo Osseo-integration (bone deposition around the implant). After the necessary radiographic interpretation abutment screw is placed to the implant body and a Dental crown is given which can be cemented or screw retained.

Is Dental Implant painful? No, it doesn’t cause pain as it is done under local anesthesia. – Who is not eligible for Implant? Age group below 18yrs, and people with unhealthy gums and underlying bone are not eligible for Implant. Uncontrolled diabetes. Poor oral hygiene. Chronic smoking habit (healing process is affected and decrease the long-term success of the implant).

2. Dental Bridge
Dental Bridge is a replacement option for missing tooth which is done by taking the support of one or more adjacent teeth. – How is it done? This procedure involves crown cutting of the adjacent teeth if the teeth are in normal condition, if not, RCT has to be done along with the crown cutting for insertion of the dental bridge which consists of pontic (replaces missing tooth) and abutments, where crowns are placed on the prepared teeth.

3. RPD
A removable partial denture is a denture for a partially edentulous patient who desires to have replacement teeth for functional or aesthetic reasons. RPD is usually not desired because of the high maintenance and also the comfort factor.

4. Do nothing!
Why we should not do anything? Doing nothing will result in the – Opposing tooth to move towards the missing tooth space which causes improper bite and malocclusions. – Surrounding teeth migrates towards the space created by the missing teeth. These are un-desirable and hence it is best to opt for one of the 3 options for missing teeth replacement from above.
